Concerning the ejector rod coming loose and falling out, believe it or not this has been a not-unheard of issue with Colts for years. I don't know if they've never heard of Loctite or??? Just degrease everything involved, apply BLUE Loctite to the rod and snug tightly. Let it rest for at least 24 hours before you shoot it again.

Last week I picked up a new 4" King Cobra...I replaced the fiber-optic front sight with a tritium insert sight. When I took the original sight off it had some lock-tite on the retaining screw threads...maybe Colt has learned from field reports of the sights blowing off Cobra snubbies early on.
