How to Replace Bolt and Hammer Screws

Can I just screw the old one out and screw the new one in? If not how does one do it? THANKS

You don't mention what you're working with, but I think it'd probably be a pretty good idea to relieve the spring tension first.

In order to do that, you'll need to strip the piece, and there are numerous guides that'll show you how to do that - including those from the NRA, that you can usually get from your local library.

Good Luck - use a well-fitted screwdriver and be patient.
Aside from having the correct screwdriver bit, you pretty well described the process. One thing about replacing "through screws" such as the hammer screw, I use a drift from the opposite side to line up parts and drive that out as the screw enters.

To remove those screws, you will need to take the gun apart:
Remove the grips, then remove the backstrap screws (3) & backstrap. Remove the mainspring, then remove the trigger guard screws (3) & trigger guard. Next remove the bolt/trigger spring screw and spring to take tension off the trigger which is pressing against the hammer. Now you can remove the hammer screw. Replace in reverse order.
